You've been asked to complete this because you have real, direct experience working alongside one of FBC's paid staff: Pastor Jake, Pastor Beth, or Kristen, and your perspective matters for giving them a thoughtful, well-rounded review this year.

This is part of FBC's regular annual review process for our paid staff. Your responses are shared only with the review committee, they are not posted publicly or shared church-wide.

We're asking for your honest, specific feedback, on their strengths and areas for growth.

It should take about 8-10 minutes to complete. If you've been asked to give feedback on more than one staff member, please complete a separate form for each.
